Support Care Coordination – Scotland

Provided by: Walking With The Wounded

Walking With The Wounded provides personalised Care Coordination for veterans, offering a single point of contact to address challenges such as housing, health, employment, debt, and social isolation.

WWTW understands that leaving military service can be challenging. Those who’ve served in the Armed Forces can face multiple and complex problems in all parts of life, including; lack of secure accommodation, debt issues, mental and physical health, unemployment, social isolation and more.

Our Support Care Coordination staff respond directly to each client and their specific needs. We discuss each area of life through a holistic needs assessment and provide a single point of contact to build a bespoke package tailored exactly to that person. Care Coordinators remain a key point of contact, helping individuals to navigate local support services to address those needs. Care Coordinators will seek out new services where there are gaps in service provision, broker conversations with support providers and support service users to attend / access support services.
